In an Emergency: If Tiptoes & Tappers are unable to contact me or an alternative care provider, in my absence, I give permission for my son/daughter to be taken to the hospital by a member of the Tiptoes & Tappers team or by ambulance. The duty paediatrician/doctor has my permission to administer treatment and/or medication as they deem necessary until such time as I am available for consultation.

Email consent: All emails between students and teachers are only to contain questions or information about dance or drama-related subjects.

Menacing email: No email may be sent that is abusive or threatens the safety of a person or persons.

Harassing email: No email may be sent that may result in the recipient suffering sexual, ethnic, religious or other minority harassment. This is based on the content of the email sent and/or its volume.

Privacy of email: No person may access or attempt to access an email intended for another user without his/her permission, except when necessary as part of that person's duties.

Spam email: No email may be sent that contains “spam” or “chain mail” content.

Language: All emails sent between students and teachers must be respectful and have no bad language.

Mailing lists: Mailing lists allow a group of people with a common interest or responsibility to communicate and share information; however, they may be open to abuse. Good practice includes: Only sending relevant emails to a mailing list. Respond only to the original email sender if other list members will not be interested in the reply. Only sending private information to relevant members of a mailing list.

Email Formatting: Format an email and use all necessary parts: addressee, subject line, salutation, body, closing, signature
 

Declaration: I understand that completion of this form and registering my child/children as a pupil of Tiptoes & Tapper and as such I undertake to arrange a Standing Order for payment of classes. I agree to give 1 month's notice if he/she no longer wishes to continue with their classes. I understand that exam entry fees and show charges are additional to class fees.

I give permission for my child’s name and medical information along with my contact number to be given to teachers for the purpose of registration.
